Jack Howell Cook Jack Howell Cook, age 75, of Butler, Tennessee, passed away on Wednesday, December 29, 2010. He was born on Todd, North Carolina on May 18, 1935 to the late Astor and Louise Howell Cook. In addition to his parents, Jack was preceded in death by a daughter, Jackie Cook and a son, David Cook. He was a member of Middle Fork Baptist Church. Jack is survived by his sisters: Vera Coffey of Hickory, North Carolina; Lois Cook of Lenoir, North Carolina; Elaine Cook of Conover, North Carolina and Janice Lea of Lenoir, North Carolina; several nieces and nephews and many special friends. The funeral service will be conducted at 11:00 a.m. on Friday, December 31, 2010 at the Mountain City Funeral Home Chapel with Rev. Lonnie Denny to officiate. Entombment will follow from the Sunset Memorial Park Mausoleum.
